The Researcher Toolkit is a library of resources, guidance, and templates for researchers, coordinators, and research staff who are involved with the development of research protocols and submissions to the IRB. All individuals who are involved in the conduct of research involving human participants are encouraged to review the information and templates below that may pertain to your studies.

IMPORTANT INFORMATION: All OUHC HIPAA Forms below were updated 5/13/2026 to reflect our campus' new name: University of Oklahoma Health Campus (OUHC). The 2026 versions are required for all new study submissions. Ongoing studies are not required to update previously approved HIPAA forms; however, study teams may update voluntarily, if enrollment is ongoing. Aside from the updated campus name, the forms' content remains unchanged from the previous version and continues to meet all regulatory and institutional requirements.
