A new version of the HSC IRB Application has been published in iRIS
Users must convert to the new form when prompted in iRIS. If after converting you find yourself in Section 5000 - Exit Application Interview, go back to the top and click 'Save and Continue to Next Section' through all sections to ensure you have answered each question. You should not lose any information that was previously entered.
