NEW! Incident Report Form for HSC is live!

Investigators reporting protocol deviations, unanticipated problems, or other events to the HSC IRB must use the new HSC Incident Report Form in iRIS. 

The Protocol Deviation Report and Unanticipated Problem (UP) Report Forms are now labeled NC to indicate that they are to be submitted to the Norman Campus IRB only. 

Any events submited to the HSC IRB on the Protocol Deviation or UP Report Forms after 8/7/2018 will be withdrawn and investigators will be asked to complete the HSC Incident Report Form.
