The OUHSC Consent Template was renamed from Form 701-G, effective 4/19/2019 to Form 701-A, effective 9/10/2019. 

This template replaces the old Form 701-A template, which is still available through the HRPP Office upon request. 

• This update is for administrative purposes only. No content changes were made. Therefore, 701-G and the new 701-A, effective 9/10/2019 are the same template.
• In lieu of modifications to update from 701-G to 701-A, the HRPP Office is recommending researchers customize and print the following note-to-file to store in their study / regulatory binder.

• The new consent template is available on the Forms & Templates page under Researcher's Toolkit.
