The following HIPAA Forms have been revised with a new version date of 08/20/2024.

• HIPAA Authorization Forms 1-4
• Request for Waiver of Authorization Form 5
• Spanish HIPAA Authorization Forms 1-4

Investigator/Study Impact:

• New studies submitted on and after 8/21/2024 must use the latest version of the HIPAA Forms.
• Studies with approved HIPAA forms continue to meet regulatory and policy requirements and therefore, DO NOT need to transition to the new versions.

The 2024 versions of the HIPAA Forms are now available in the Researcher Toolkit and in the Help section of iRIS.
