New Consent Template, 701G, available on the Forms & Templates page, effective 3/12/2019. This version replaces the 12/20/2018 version.  Researchers can also access the template in the Help section of iRIS.

IMPORTANT NOTES:

• Minor updates and changes were made to template instructions. No changes have been made to any required template language. 

• Delete any stored copies you may have of the consent version 12/20/2018 and replace it with the new 03/12/2019 version.

• Researchers that have already have an approved RCR consent form (on the 12/20/18 version) do NOT need to transition to this new 3/12/2019 version.
