A new version of the Annual Check-In Form has been published to add study-specific general information, including the IRB number, study title, and PI/study contact names. The information will automatically populate in Section 1.0 of the form as Question 1.1. Including this information in the Check-In Form will enhance and simplify record-keeping efforts for research teams.

All users creating a new Check-In Form or who are in the middle of completing a Check-In Form will convert to the new version.
