Effective 12/8/2023, a required template has been published for survey studies that will request a waiver of signed consent. Studies submitted on and after this date, as well as survey studies currently under review and pending IRB approval, will be expected to use this template. IRB staff will coordinate with investigators on studies that are pending final IRB approval. Ongoing and previously approved survey studies are not required to use this new template. Additionally, new guidance for submitting survey studies to the IRB has been published. This guidance provides recommendations and guidelines for how to prepare your documents and materials before submitting to the IRB for approval.
