A new HSC Consent Template version is available, dated 9/28/21. The main change to the template is the addition of language for legally authorized representatives (LAR) starting at line 69. Additional minor changes were made to improve clarity and understanding. Investigators are not required to transition existing consent forms to the new template. However, investigators are expected to use this revised template on new studies moving forward.  The consent template is available here.
